It ends with a Creed, a Salve Regina and the following:

PRAYER

Prodigious saint, glorious Saint Judas Thaddeus, honor and glory of the apostolate, relief and protection of the afflicted sinners, I ask you for the crown of glory you have in heaven, for the singular privilege of being a close relative of Our Savior and for the love you had to the Holy Mother of God, to grant me what I ask of you. Just as I am sure that Jesus Christ honors you and grants everything, so may I receive your protection and relief in this urgent need.

CONCLUSIVE PRAYER

(in difficult cases)

O glorious St. Jude Thaddeus, the name of the traitor who placed his adorable Master in the hands of his enemies has caused you to be forgotten by many. But the Church honors you and invokes you as a lawyer for difficult things and desperate cases.

Pray for me, so miserable; make use, please, of that privilege which the Lord granted you: to bring quick and visible help in those cases in which there is almost no hope. Grant that in this great need I may receive, through your mediation, the relief and consolation of the Lord and may also in all my pains praise God.

I promise to be grateful to you and to spread your devotion to be eternally with God with you. Amen.

His bond with Jesus

Judas Thaddeus was born in Cana of Galilee, Palestine, son of Alphaeus (or Cleofa) and Maria Cleofa. His father Alfeo was brother of San Giuseppe and his mother cousin of Maria Santissima. Hence Judas Thaddeus was Jesus' cousin, both from father and mother. Alphaeus (Cleophas) was one of the disciples to whom Jesus appeared on the way to Emmaus on the day of the resurrection. Maria Cleofa was one of the pious women who had followed Jesus since Galilee and who remained at the foot of the cross, in Calvary, together with Mary Most Holy.

Judas Thaddeus had four brothers: Giacomo, Giuseppe, Simone and Maria Salome. One of them, James, was also called by Jesus to be an apostle. The relationship of the family of St. Jude Thaddeus with Our Lord Jesus Christ himself, from what is possible to perceive from the Holy Scriptures, is as follows. Among the brothers, James was one of the twelve apostles and became the first bishop of Jerusalem. We know that Giuseppe was known as the Just One. Simone, another brother of San Giuda, was the second bishop of Jerusalem, successor of Giacomo. Maria Salome, the only sister, was the mother of the apostles San Giacomo Maggiore and San Giovanni Evangelista. He was called Giacomo Minore to distinguish himself from another apostle, San Giacomo, who being bigger was called Maggiore.

There is supposed to have been much coexistence between St. Jude Thaddeus, his cousin Jesus and his uncles Mary and Joseph. It was certainly this fraternal coexistence, in addition to the very close relationship, that led St. Mark (Mk 6: 3) to quote St. Jude Thaddeus and his brothers as "brothers" of Jesus.
